About this role

Walker Meadow Retirement Community's LifeCircles PACE program is seeking an RN Clinical Coordinator to deliver nursing care across multiple settings—clinic, participants' homes, and care facilities—for frail elderly individuals. You'll conduct assessments, manage medication oversight, administer treatments, and develop individualized care plans as part of an interdisciplinary team. The role demands strong clinical judgment and the ability to work independently while coordinating complex care needs and transitions between settings.

This position requires a current Michigan RN license and at least one year of experience with elderly or long-term care populations; home care or long-term care background is preferred. You'll spend your time split between the PACE center in Muskegon and community settings, working Monday through Friday, 8am to 4:30pm, with compensated on-call rotation as part of the exempt schedule. The role suits nurses who value direct participant relationships, thrive in collaborative team environments, and are comfortable navigating the clinical and logistical complexities of coordinated care.

The position offers six weeks of flexible time off annually plus paid holidays, medical/vision/dental insurance beginning after 30 days, a retirement plan with employer contributions, daily pay options, mileage reimbursement, and up to $3,000 annually in tuition assistance for professional development.
