About this role

Connections Health Solutions operates crisis stabilization services across Arizona, and this Care Coordinator role sits at the center of that work. You'd manage intake assessments, treatment planning, and discharge coordination for individuals in behavioral health crisis, working alongside physicians, therapists, and community partners to move people from acute crisis toward recovery. The role demands strong clinical judgment—you'll conduct psychosocial assessments, develop individualized treatment plans, identify service gaps, and maintain regular communication with patients and providers to ensure continuity of care.

The position requires a bachelor's degree in social work or a related behavioral health field, or an associate's degree paired with at least two years of experience in acute or transitional outpatient settings. Spanish fluency and a master's degree are valued but not required. This is a full-time, in-office role in Tucson with comprehensive benefits including health insurance, 401k matching after six months, and 160 hours of PTO annually. Vaccination is mandatory as a condition of employment.

This role suits someone comfortable with clinical documentation, case management, and coordinating across multiple stakeholders in a fast-paced crisis care environment. You'd need to balance thorough assessment work with frequent communication demands and the ability to work collaboratively within an interdisciplinary team.
