About this role
UP is hiring an LPN Care Coordinator to work in-office in Cleveland supporting their behavioral health initiative. The role centers on chart review and care coordination for residents in skilled nursing and long-term care facilities, requiring you to assess patients' medical, social, and emotional needs, monitor progress, adjust care plans, and collaborate with clinical teams to reduce gaps and hospitalizations. You'll spend significant time in the EMR system, reviewing records for accuracy and completeness, while also conducting patient assessments and vital sign monitoring.
The position calls for an active LPN license and at least two years in acute or managed care, with geriatric behavioral health experience preferred. You should be comfortable with extended computer work and the physical demands of patient care—lifting, moving, and prolonged sitting. Strong communication skills and the ability to work across interdisciplinary teams are essential.
This full-time role runs 9:00 am to 5:30 pm and pays $30–$34 per hour, with health, dental, and vision coverage, 401K, company life insurance, and paid time off. It's suited to an LPN looking for a stable, mission-focused position in senior care without the shift variability of facility-based nursing.
