You'll manage member caseloads across the seven-county metro area, conducting assessments, coordinating care transitions, developing individualized care plans, and connecting members with community resources. The role balances field work—roughly 50 to 70 percent travel to assisted living facilities, group homes, and skilled nursing settings—with remote work from your home office. You'll collaborate with multidisciplinary care teams to ensure quality outcomes and help members navigate complex health and social needs.

This position requires a Minnesota Licensed Social Worker or Registered Nurse license, with at least one year of case management or care coordination experience in geriatric or disability services preferred. You'll need a valid driver's license, strong communication and problem-solving skills, and the ability to work independently while maintaining a compliant home office. The role suits someone comfortable with diverse populations and aligned with Bluestone's values of dedication, collaboration, and caring.

The full-time schedule runs Monday through Thursday, 8am to 5pm, and Friday, 8am to 3pm—no evenings, weekends, or holidays. Compensation ranges from $65,000 to $75,000 based on experience. Benefits include health, dental, and vision coverage; company-paid life insurance and disability coverage; a 4 percent retirement match; three weeks paid time off plus nine paid holidays; mileage reimbursement; and a company phone and laptop.
