Description
- Implements, evaluates, and monitors patient care plans, documenting and communicating changes in patient condition as appropriate.
- Coordinates care among interdisciplinary team members to support effective, well-coordinated patient outcomes.
- Reviews and carries out physician and nursing orders in accordance with established protocols.
- Assesses patient discharge needs and collaborates with the healthcare team to develop and coordinate appropriate discharge plans.
Qualifications
Licensure / Certification / Registration:
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
Education:
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
