About this role

BAYADA's float pool model offers a streamlined alternative to traditional full-time home health practice. You would conduct 10–12 shifts monthly across Northeast Philadelphia and Lower Bucks County, evaluating patients post-discharge and designing individualized therapy plans that span mobility assessment, gait training, balance work, and wound care protocols. The role emphasizes continuity—following patients from hospital transition through to safe, independent home functioning—while collaborating with nursing and other disciplines on care adjustments.

This position suits experienced home health clinicians comfortable with autonomy and variety. You'll need an active Pennsylvania PT license and prior OASIS and home health billing experience. The work demands strong assessment and communication skills, reliable transportation between visits, and the interpersonal presence to build trust quickly with patients in their own environments. BAYADA offers premium rates plus mileage reimbursement to offset the travel inherent in float assignments, and the limited monthly commitment allows you to maintain other work or personal priorities alongside this role.
