About this role

Theoria Medical is looking for an Advanced Practice Provider to deliver ongoing care to residents across skilled nursing facilities in the Plainwell area. You'd conduct daily rounds, manage medications and treatment plans, handle acute issues as they arise, and coordinate with facility nursing staff—all within predictable 6-hour shifts that typically start between 7 and 10 a.m., with no on-call or overnight duties. The role emphasizes continuity of care and building relationships with vulnerable populations over time.

The position requires an active U.S. license and board certification (ANCC, AANPCB, PNCB, or AACN for NPs; NCCPA for PAs), with preferred backgrounds in Internal Medicine, Family Medicine, Emergency Medicine, Geriatric Medicine, or related specialties. You'll work within Theoria's technology ecosystem—an EHR designed for efficiency, voice dictation support, and an iPad for charting—alongside a collaborative clinical team. The company emphasizes flexibility and autonomy in how you practice.

Compensation is set at $50,000 annually, with additional earning potential through annual performance bonuses and longevity incentives tied to long-term commitment. The full benefits package includes PTO, 401(k) matching, medical/vision/dental coverage, disability insurance, CME reimbursement, and employer-paid life insurance. This role suits providers seeking stable, predictable scheduling and the opportunity to build deep relationships with patients in a post-acute care setting.
