About this role
WelbeHealth's PACE program in San Jose is looking for a Personal Care Assistant to join an interdisciplinary team supporting vulnerable seniors who remain in their communities. You'd work at a day center providing hands-on assistance with daily living activities—bathing, dressing, toileting, transfers—while also taking vital signs, escorting participants to appointments, and helping oversee their safety and meals throughout the day. This isn't traditional facility-based care; you'd be part of a collaborative environment where seniors access medical appointments, rehabilitation, and social activities all in one setting.
The role requires a high school diploma and at least one year of personal care experience with seniors. While HHA or CNA certification is preferred, it's not mandatory to apply. You'll need a valid California driver's license to transport participants to medical appointments, and you must obtain CPR and First Aid certification within six months of hire. A Food Handlers Certificate is required within 30 days. The position pays $23.76 to $28.62 per hour plus bonus, with medical, dental, and vision coverage, 401(k) matching, and 35 days of combined PTO and holidays annually.
This role suits someone starting or building a healthcare career who values direct participant interaction and teamwork across departments. WelbeHealth emphasizes work-life balance and requires COVID-19 vaccination as a condition of employment.
