About this role

FronteraCare is seeking an LCSW to lead its Community Health Worker team across New Mexico and Colorado, overseeing day-to-day operations for a remote, regionally dispersed group. You'd manage CHW performance and family engagement while ensuring alignment with state standards and the organization's clinical protocols for autism diagnostic and ABA therapy services. The role bridges CHWs, clinical leadership, and administration, requiring you to monitor intake metrics, intervene directly with at-risk families, and drive continuity of care across the organization's regions.

Your responsibilities span clinical oversight, team leadership, and operational problem-solving. You'll conduct regular supervision and case reviews, provide coaching and professional development to CHWs, and identify systemic bottlenecks in the intake process. You'll also model high-quality family engagement yourself, stepping in when complex cases need leadership input or when families have stalled in the pipeline. Technology competency matters here—you'll ensure the team is proficient with FronteraCare's platforms, including consent workflows and parent training apps.

This position suits someone with at least three years of experience in community health work, care coordination, or behavioral health, plus an active social work license and demonstrated peer leadership experience. You should be comfortable managing remote teams, analyzing performance data to spot trends, and working collaboratively across departments to solve operational challenges. The salary range is $65,000 to $80,000.
