About this role
Ascension's LPN Patient Navigator role in Tulsa centers on bridging clinical care and patient experience within a busy clinic setting. You'll handle the full patient intake workflow—collecting vitals, documenting health histories, and running preventative screenings—while also delivering direct clinical care including medication administration, immunizations, wound care, and assistance with minor procedures. Beyond the clinical floor, you'll manage the coordination that keeps care continuous: triaging patients, communicating lab results, processing referrals, and ensuring nothing falls through the cracks.
The role demands a current LPN license and ideally BLS certification, though Ascension will consider equivalent experience in lieu of formal education. You'll need strong clinical fundamentals and the ability to move fluidly between hands-on patient care, patient education on chronic disease management and self-care, and the administrative follow-up that drives compliance and long-term outcomes. This is a full-time, in-office position suited to someone who values both direct patient interaction and the operational details that make a clinic run smoothly.
