About this role

BAYADA's Transition Team in Philadelphia bridges hospital and home care for patients with complex medical needs, particularly those with rare diseases cared for through partnerships like CHOP. As a weekend RN on this team, you'd manage the critical handoff period—delivering direct care while training the ongoing home care team to take over. The role demands strong clinical skills in high-acuity settings; you'll need current NICU, PICU, ICU, or trach/vent home care experience, plus the ability to teach and prepare other caregivers for continuity of care.

Weekend availability is the core requirement here, with current openings for overnight shifts (Friday through Sunday nights) and day shifts (Saturday and Sunday days), though weekday overnight options exist too. The position is part-time and hourly, paying $40–$45 per hour for Transition Team roles depending on your qualifications and schedule commitment. You'll need a current RN license, CPR certification, willingness to travel within Philadelphia County, and the capacity to maintain a consistent schedule.

BAYADA offers the infrastructure you'd expect from an established home health provider: weekly pay, comprehensive medical benefits, paid training, tuition reimbursement, 401(k) matching, and a supportive local office environment. This role suits nurses who thrive in teaching-focused, patient-centered work and want the flexibility of weekend-based scheduling without sacrificing stability or professional development.
