About this role
BAYADA's Transition Team in Philadelphia is looking for LPNs to work overnight shifts supporting patients moving from hospital or intensive care settings into home-based care. You'd deliver direct care to infants, children, and adults with complex medical needs—many with rare diseases—while training the ongoing care teams who will take over long-term. This role pairs clinical responsibility with education, requiring you to prepare and mentor other caregivers on specialized protocols and techniques.
The position demands strong ICU or pediatric critical care background, specifically NICU, PICU, or experience with tracheostomy and ventilator-dependent patients. You'll need current LPN licensure, CPR certification, and the ability to pass high-tech adult and pediatric simulations. The overnight schedule runs Monday through Sunday with part-time availability; hourly pay ranges from $30 to $42 depending on qualifications, with Transition Team rates at the higher end ($40–$42). BAYADA offers weekly pay, comprehensive medical benefits, paid time off, industry-funded training, and tuition reimbursement.
This role suits LPNs who thrive in teaching environments and want to work with medically complex populations in a home care setting rather than a hospital. You'll need reliable transportation throughout Philadelphia County and the commitment to maintain a consistent schedule. If you have the clinical depth and patience to both deliver care and develop others, this is a direct entry point into BAYADA's established home health network.
