    About this role

Leading a case management team at Ascension's Brookfield location means overseeing both the clinical and operational sides of post-acute care coordination. You'll manage professional and clerical staff while handling utilization review, discharge planning, and disease management—working across hospital administration and medical teams to ensure appropriate admissions and lengths of stay. The role requires you to be both a hands-on manager and a subject matter expert, coaching staff on appeal processes and documentation standards while covering units directly when complex cases demand it.

This position calls for a Wisconsin-licensed RN or social worker with at least three years of relevant experience and one year in a leadership capacity. A case management certification from ACMA or CCMC is preferred. You'll develop schedules, redistribute workload daily to maintain coverage, and serve as the go-to resource for your team on utilization criteria and case management systems. A master's degree strengthens your candidacy, though an associate or technical degree paired with sufficient experience meets the baseline requirement.

The role is full-time and based in-office in Brookfield. Ascension, a large nonprofit Catholic health system, emphasizes service and growth in its culture and has been recognized as a top healthcare employer.
