Description

Occupational Therapist (OT) — Home Health | $10K Sign-On Bonus

Full-Time / Part-Time | San Francisco & San Mateo County
$60–$75/hour | $10,000 Sign-On Bonus

Full-time means a full-time paycheck. Our therapists are paid hourly for their scheduled hours, including drive time and documentation. No per-visit pay, no unpaid charting, and a growing patient census that keeps your schedule steady.

At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We're growing our team and looking for an OT who wants meaningful work, modern tools, and a team that's truly there for them.

You'll be assigned one dedicated territory, either San Francisco (full-time) or San Mateo County (part-time), designed to minimize drive time and keep your day manageable. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City.

Why OTs Choose Health Link

- Reliable income: Many agencies promise full-time work but only pay per visit, so a light week means a smaller paycheck. At Health Link, full-time staff are paid hourly for their scheduled hours, and our growing census means a steady, consistent caseload.
- Hourly pay that covers your whole day: A long visit, a tough drive, or a complex note never costs you money.
- Earn up to 25% more: On top of your hourly pay, earn daily, per-payroll, and quarterly bonuses based on completed visits and use of our AI-assisted documentation.
- AI-assisted charting: Smart documentation tools on company devices that cut charting time and keep your evenings free.
- Real-time support in the field: Our experienced case managers and clinical team are available in real time to help with complex situations or patient coordination, so you're never on your own.
- Easy commute: Mileage reimbursement, a company vehicle (based on availability), or rideshare credits in the SF territory.
- A manageable pace: Organized scheduling, territories designed to minimize drive time, and a team that respects your time.

About this role

Health Link Home Health & Hospice is hiring occupational therapists for direct-hire positions serving San Francisco or San Mateo County. You'd work within a single assigned territory, minimizing commute time while building a steady caseload of home health patients. The role involves standard OT responsibilities—evaluations, treatment planning, skilled interventions focused on ADLs and home safety, and coordination with the care team—but with a key structural difference: you're paid hourly for all scheduled time, including drive time and documentation, rather than per visit. This means your income stays consistent even during lighter weeks, and you won't be charting unpaid into the evening.

The compensation reflects an effort to retain experienced clinicians. The base range is $60–$75 per hour depending on experience, plus a $10,000 sign-on bonus. You can earn productivity bonuses up to 25% of wages through completed visits and use of the company's AI-assisted documentation system. Health Link covers mileage, offers company vehicles or rideshare credits in SF, and provides standard benefits including medical, dental, vision, 401(k) matching, and paid time off.

This role suits OTs with a current California license and valid driver's license who want predictable income and manageable scheduling. Home health experience is preferred but not required; strong outpatient, SNF, or hospital backgrounds are welcome, with orientation provided. The company emphasizes real-time clinical support and modern tools, positioning itself as an alternative to the per-visit model common in home health staffing.
