About this role
Remedy Home Health Care is looking for a licensed Physical Therapist to deliver skilled rehabilitation services to senior and geriatric patients across Los Angeles County—primarily in the Huntington Park, Maywood, and South Gate areas. The role offers flexibility through per diem, part-time, or full-time scheduling, with compensation of $120–$150 per visit plus mileage reimbursement. You'll conduct evaluations, design personalized care plans, and provide hands-on therapy focused on fall prevention, strength, balance, and functional mobility, working in patients' homes, assisted living facilities, and community settings.
This position suits PTs with at least one year of clinical experience, ideally in home health or geriatrics. You'll need a California PT license, current CPR certification, a valid driver's license, reliable transportation, and auto insurance. The role emphasizes autonomy and direct patient impact—you'll work independently in the field while collaborating with nurses, case managers, and physicians. Strong communication and patient education skills are essential, as you'll teach families about safe mobility and fall prevention alongside clinical care.
If you're drawn to one-on-one rehabilitation work with older adults and value schedule flexibility and community-based practice over a traditional clinic setting, this offers meaningful work with a mission-driven organization across a large service area.
