Description
Career Opportunity – Care Manager with Found Village in Cincinnati, OH
Are you a collaborative, system-minded professional who believes every young person deserves a connected village working toward their future? Do you build trust across organizations, bring clarity to complexity, and anticipate needs before they become barriers? If so, we want to speak with you!
The Care Manager creates the alignment that allows the village surrounding each young person to work as one connected team. While Relationship Coaches focus on developing the young person, the Care Manager develops the network around them, bringing caregivers, schools, providers, employers, community organizations, and natural supports together around shared goals, clear roles, and coordinated action.
Responsibilities:
- Build trusted relationships with young people, coaches, caregivers, schools, providers, employers, community partners, and natural supports.
- Create shared understanding of each young person’s goals, developmental priorities, barriers, next steps, and measures of success.
- Develop and continuously refine collaborative developmental plans with meaningful youth participation.
- Establish initiative-taking communication rhythms that create clarity, accountability, shared ownership, and action.
- Coordinate services, opportunities, resources, and transitions so young people experience one connected developmental journey.
- Navigate partnerships across healthcare, education, child welfare, employment, housing, behavioral health, transportation, and community organizations.
- Anticipate barriers before they become crises and mobilize the people and resources needed to maintain momentum.
- Help stakeholders understand how their role reinforces coaching and advances youth development.
- Use documentation, outcomes, data, and stakeholder feedback to improve individual coordination and organizational systems.
- Support young people in increasingly understanding, influencing, and leading conversations about their own future.
Requirements:
- Experience coordinating multiple stakeholders and building collaborative partnerships without relying on formal authority.
- A systems-oriented approach and the ability to see how people, services, and opportunities can work together.
- Strong facilitation and communication skills, including the ability to navigate honest or difficult conversations.
- A demonstrated commitment to keeping young people at the center of planning and decision-making.
- The ability to balance empathy with accountability and bring calm, clarity, and follow-through to complex situations.
- Strong organization, responsiveness, documentation, and continuous-improvement practices.
- An initiative-taking mindset with the ability to anticipate needs, reduce service gaps, and coordinate smooth transitions.
- A belief that coordination is an act of care and that young people should not carry the burden of coordinating the adults and organizations designed to support them.
- Strong judgment and decision-making, with comfort navigating ambiguity.
- Care coordination and planning, paired with relationship-building and the ability to direct action.
- Ability to collaborate and lead across teams.
What success looks like:
- Young people understand their developmental goals, actively participate in planning, navigate transitions with confidence, and experience fewer service gaps.
- Caregivers, coaches, providers, schools, and partners share priorities, understand their roles, communicate proactively, and respond quickly to changing needs.
- Coaches can spend more time developing young people because coordination is functioning effectively.
- The village reinforces shared developmental goals instead of creating competing plans or fragmented interventions.
- Community partners experience Found Village as collaborative, organized, responsive, and dependable.

A System in Need of Support
We fill the gaps.
Let’s face it, our society runs on a network of various systems set in place to meet the needs of individuals. Unfortunately, many of these systems, though well intended, are not designed to raise kids and meet their needs holistically. Because of this, the teens we work with often lose trust, give up, and let their pain drive choices and behavior. They have been in and out of detention centers, courtrooms, foster homes, shelters, different schools, and more. They are from hard places and circumstances that leave them frustrated and discouraged. They are assigned diminishing labels, misdiagnoses, unrealistic work plans, and offered subpar support programs. To nobody’s surprise, there is often no real change.
We come alongside families and communities in need. We are here to partner with these systems—to come behind them and fill in the gaps wherever necessary. Without a holistic approach to serving kids in crisis, many areas of need are left unmet. In turn, this results in deeper levels of trauma, neglect, and misunderstanding. We exist to support our teens and their families so they can understand and navigate the systems they have become a part of. We aim to help elevate the voices of our youth and provide them with the support that an average teenager born into different circumstances would inherently receive.

About this role
Found Village is seeking a Care Manager to serve as the connective tissue between young people and the multiple systems designed to support them—schools, healthcare providers, employers, child welfare agencies, and community organizations. Rather than focusing on direct coaching, you'd spend your time building trusted relationships across these stakeholders, translating their separate priorities into shared goals, and coordinating action so young people experience seamless support instead of fragmented services. This role demands someone who can facilitate difficult conversations, anticipate barriers before they escalate into crises, and maintain clarity and accountability across complex partnerships without relying on formal authority.
Success in this position requires both systems thinking and relational skill. You need strong judgment to navigate ambiguity, the initiative to spot service gaps and mobilize resources, and the ability to balance empathy with accountability. Documentation, responsiveness, and continuous improvement matter, but they serve a larger purpose: ensuring that young people understand their own goals, participate actively in planning, and gradually take ownership of their future rather than being managed by the adults around them. The role is grounded in the belief that coordination itself is an act of care.
This is a full-time, in-office position in Cincinnati, Ohio with strategic HR inc. It suits someone with demonstrated experience coordinating across multiple stakeholders and building partnerships, who sees young people as central to their own development, and who finds meaning in systems-level work that reduces burden on the individuals those systems are meant to serve.
