About this role
Ascension's Patient Navigator role bridges clinical care with community support, positioning you as a key connector among hospital management, physicians, insurers, and local organizations. You'll coordinate treatment plans, track clinical outcomes to drive quality improvements, and help staff stay current through educational initiatives. The work centers on ensuring vulnerable populations—particularly uninsured and underinsured patients—access the follow-up care, financial assistance, and community resources they need.
This position requires a Tennessee-licensed RN and a nursing diploma or equivalent, with BLS certification preferred. Based in Nashville and working on-site full-time, you'll combine clinical knowledge with care coordination and advocacy skills. It's well-suited to nurses who want to move beyond bedside care into systems-level problem-solving and community health, especially those drawn to serving populations with complex social and financial barriers to care.
