About this role
Ascension's Patient Navigator role in Nashville centers on connecting people with healthcare services and supporting them through the process. You'll identify clients who need assistance, conduct initial assessments, and then guide them—sometimes accompanying them to appointments, helping with applications, and providing education and informal counseling along the way. The work involves substantial coordination between internal teams and external community resources, so strong relationship-building skills and attention to detail in documentation are essential.
This is an in-office, full-time position suited to someone with a genuine interest in social support and healthcare access. While a BLS Provider certification and driver's license are preferred, the role doesn't require clinical credentials. You'll need to be organized, comfortable with follow-up work, and able to maintain accurate records to track client outcomes. If you're drawn to direct service work that helps people navigate a complex system, this could be a good fit within a large nonprofit health system.
