About this role

Ennoble Care is seeking a per diem Nurse Practitioner to conduct face-to-face hospice assessments for patients across the Atlanta region. Working remotely with flexibility around visit scheduling, you'll perform the clinical evaluations required by Medicare and other payers to establish and recertify hospice eligibility. The role demands careful documentation that clearly supports continued decline—using objective measures like PPS and FAST scores, weight trends, and functional status—while avoiding generic language that won't withstand regulatory scrutiny.

Your day involves reviewing patient histories before each visit, identifying eligibility indicators and symptom burden, then conducting assessments that inform the broader hospice team's care planning. You'll reconcile medications, educate families on disease progression and goals of care, and communicate recommendations to the medical director and interdisciplinary team. Same-day documentation is expected, with clear clinical findings and decline indicators that justify physician recertification.

This role suits an experienced NP comfortable working within a physician-supervised model and familiar with hospice regulations and documentation standards. Ennoble Care operates across ten states, so you'd be part of a larger mobile primary and palliative care organization, though your focus is the Atlanta market. Full-time employees receive standard benefits including medical, dental, vision, paid time off, and a 401(k) match; all staff get paid sick time and access to Payactiv.
