Built for nurses who want autonomy — not isolation.
Full-time means a full-time paycheck. Our nurses are paid hourly for their scheduled hours, including drive time and documentation. No per-visit pay, no unpaid charting, and a growing patient census that keeps your schedule steady.
At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We're growing our San Francisco team and looking for a compassionate, organized RN Case Manager.
Whether you're a seasoned home health nurse or a hospital nurse looking for a change of pace, we offer a structure that respects your license, rewards your expertise, and supports you in the field.
Why SF Nurses Choose Health Link
- Reliable income: Many agencies promise full-time work but only pay per visit, so a light week means a smaller paycheck. At Health Link, full-time nurses are paid hourly for their scheduled hours, and our growing census means a steady, consistent caseload.
- Hourly pay that covers your whole day: A long visit, a tough drive across the city, or a complex note never costs you money.
- Mileage reimbursement and a company vehicle (based on availability) are available.
- AI-assisted charting: Smart documentation tools on company-issued iPads that cut charting time and keep your evenings free.
- Real-time support: Our clinical leadership and intake teams are available in real time when you need orders, coordination, or a second opinion. You're never on your own.
- A manageable pace with organized scheduling and a team that respects your time.

About this role
Health Link Home Health & Hospice is seeking an RN Case Manager to join their growing San Francisco team, managing a steady caseload of home health patients while maintaining genuine work-life balance. You'd handle the full scope of skilled nursing in patients' homes—OASIS assessments, medication administration, wound care including complex dressings, IV therapy management, and care coordination with physicians and therapy teams. The role emphasizes clinical autonomy paired with real-time support from leadership, so you're never problem-solving alone despite working independently in the field.
What sets this position apart is how it's structured around clinician sustainability. You're paid hourly for all scheduled time, including drive time and documentation, which means your paycheck stays consistent even during lighter weeks. The agency provides rideshare credits for SF visits, company iPads with AI-assisted charting to minimize evening paperwork, and flexible scheduling that respects your time. This appeals equally to seasoned home health nurses and hospital or SNF nurses ready to transition into a less fragmented pace.
You'll need an active California RN license, a year of nursing experience minimum, and reliable transportation. Home health background is preferred but not required—the organization trains strong acute-care nurses in home health protocols and OASIS. The role includes a $10,000 sign-on bonus, hourly pay ranging from $64.90 to $75.00 based on experience, and standard benefits including medical, dental, vision, 401(k) matching, and paid time off.
