Description

Physical Therapist (PT) — Home Health | $15K Sign-On Bonus!

Full-Time / Per Diem | San Francisco, CA
$70–$90/hour | $15,000 Sign-On Bonus

Full-time means a full-time paycheck. Our therapists are paid hourly for their scheduled hours, including drive time and documentation. No per-visit pay, no unpaid charting, and a growing patient census that keeps your schedule steady.

At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the Bay Area with a focus on clinical quality, strong systems, and a collaborative culture. We're growing our San Francisco team and looking for a PT who wants meaningful work, modern tools, and a team that's truly there for them.

You'll see patients across SF's neighborhoods, from the Sunset and Richmond to the Mission and beyond, with a territory designed to minimize drive time and keep your day manageable.

Why PTs Choose Health Link

- Reliable income: Many agencies promise full-time work but only pay per visit, so a light week means a smaller paycheck. At Health Link, full-time staff are paid hourly for their scheduled hours, and our growing census means a steady, consistent caseload.
- Hourly pay that covers your whole day: A long visit, a tough drive across the city, or a complex note never costs you money.
- Earn up to 25% more: On top of your hourly pay, earn daily, per-payroll, and quarterly bonuses based on completed visits and use of our AI-assisted documentation.
- AI-assisted charting: Smart documentation tools on company devices that cut charting time and keep your evenings free.
- Real-time support in the field: Our experienced clinical team can help with a new situation, call a patient when you're running late, or handle a lab result, so you're never on your own.
- SF-friendly commute: Mileage reimbursement, a company vehicle (based on availability), or rideshare credits.
- A manageable pace: Organized scheduling, territories designed to minimize drive time, and a team that respects your time.

About this role

Health Link Home Health & Hospice is seeking a physical therapist to join their growing San Francisco team, where you'd deliver skilled care to patients across the city's neighborhoods while maintaining a sustainable schedule. You'll conduct evaluations, develop treatment plans focused on mobility and fall prevention, and educate patients and caregivers in their homes—all supported by AI-assisted charting tools and real-time clinical backup from an experienced team. The role includes supervising PTAs when assigned and coordinating with physicians and case managers as part of an interdisciplinary approach.

What sets this position apart is the compensation structure: you're paid hourly for all scheduled time, including drive time and documentation, with no per-visit model that leaves you unpaid during lighter weeks or complex cases. The $64.90–$91.35/hour range translates to $135K–$190K annually for full-time staff, plus a $10,000 sign-on bonus. Health Link covers mileage reimbursement, offers company vehicles or rideshare credits, and provides standard benefits including medical, dental, vision, 401(k) matching, and paid time off.

You'll need a current California PT license and valid driver's license; home health experience is preferred but not required, as the company welcomes strong outpatient, SNF, or hospital therapists and provides orientation and OASIS training. The role offers flexibility—choose full-time or per diem—with scheduling designed to minimize commute time across San Francisco. This is a direct hire position, not contract work.
