About this role
Luminis Health is seeking an RN to build and lead transitional care pathways for vulnerable pediatric and adult patients navigating between inpatient, outpatient, and community settings. You'd work across clinical teams, families, and external organizations—nursing facilities, home health agencies, hospice providers, and community groups—to design protocols and coordinate treatment plans that keep at-risk patients connected to the resources they need. The role blends clinical assessment, care coordination, and program development, requiring you to identify gaps, educate patients and families, and serve as a clinical resource to the broader care management team.
This position suits an RN with at least three years of general medicine experience, ideally spanning both inpatient and outpatient environments, who thrives in cross-functional collaboration and enjoys building systems rather than following them. A BSN is preferred. You'll need a current Maryland nursing license and BLS certification. The work is based in Luminis Health's Lanham office and demands the creativity and energy to launch a targeted program from the ground up.
Luminis Health offers standard benefits including medical, dental, and vision coverage, a retirement plan with employer match, paid time off and holidays, tuition assistance, and wellness programs. The salary range is $35.04 to $52.55 per hour.
