Description
Health Link Home Health & Hospice is seeking an experienced Licensed Vocational Nurse (LVN) to join our growing Transitions Department.
The Transitions Nurse works closely with our Home Health clinical teams to identify patients whose healthcare needs are changing and help connect them with the appropriate level of care, services, and support. This may include hospice, palliative care, additional caregiving support, higher levels of care, placement, or other community resources based on the patient’s individual needs.
The Transitions Nurse serves as a consistent resource for patients and families throughout these changes, providing education, guidance, coordination, and follow-up to help ensure patients receive the right care at the right time.
A key component of the role includes recognizing patients who may benefit from hospice evaluation, providing hospice education and informational visits, and coordinating with the clinical team to facilitate timely transitions when appropriate.

About this role
Health Link Home Health & Hospice's Transitions Department needs an LVN who can bridge the gap between home health care and the next appropriate level of support for patients whose needs are shifting. You'd spend your time identifying when patients require hospice, palliative care, additional support, or placement; educating families about their options; and coordinating the handoff to ensure continuity of care. This role sits at the intersection of clinical assessment and care navigation—you're both a clinician and a guide through a sensitive transition.
The work combines field-based patient visits with collaboration across Home Health teams, hospice staff, physicians, and community partners. You'll need to recognize subtle changes in patient condition, communicate those concerns clearly to clinical leadership, and then support families through the emotional and practical realities of moving to a different care setting. Strong communication skills matter as much as clinical judgment here, since much of your impact comes through education and conversation rather than direct patient care tasks.
You'll need at least a year of nursing experience with preference given to backgrounds in home health, hospice, palliative care, or care coordination. The role requires a valid California LVN license, reliable transportation, and comfort working independently while staying connected to a larger team. The position is full-time, based in San Francisco with both office and field work, and pays $40–$45 per hour with standard benefits including health insurance, 401(k) matching, and flexible scheduling.
