About this role
Walker Meadow Retirement Community's PACE program in Southwest Michigan is seeking a Registered Dietitian to deliver nutrition care to older adults across two locations (Niles and St. Joseph). This per diem, full-time role combines clinical assessment and care planning with community-based work—you'll conduct nutrition evaluations, develop individualized treatment plans for chronic disease management, lead interdisciplinary team meetings, and make home visits to evaluate food access and meal preparation capacity. The position also includes oversight of the PACE kitchen and monitoring of meal compliance and dietary adherence.
You'll need an RD or RDN credential, a bachelor's degree minimum in nutrition or dietetics, and at least one to two years of clinical experience preferably in geriatrics or long-term care. One year working directly with frail and elderly populations is required. A valid driver's license and reliable transportation are essential given the home visit component. ServSafe Food Protection Manager certification is required within six months if not held at hire; a Certified Diabetes Educator credential is desired but not mandatory.
The role offers tuition assistance, a retirement savings plan with employer contributions, $120 annual wellness reimbursement, mileage reimbursement for work-related travel, and daily pay options. A $500 referral bonus is available if you bring new team members to the organization.
