Join our team and make a positive impact on the lives of others! We are looking for a Care Manager for our team in Indiana.
This is a field-based role with required travel throughout the South Bend, IN area.
Program & Position Overview:
The Care Manager will be a vital part of our operations in Indiana. Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they provide the best service possible to their loved ones.
Every Day You Will:
- Evaluate the home care needs of the client
- Assess the environment
- Fitness of the caregiver to provide the care
- Home orientations involving signing of paperwork
- Initial assessments during orientations
- Periodic supervisory visits / check in for on-care cases
- Training caregivers / patients as required per case
- Interacting with the care manager for the patient as needed
- Engaging in interdisciplinary meetings with the care managers, caregivers and patients
- Collaborate in care and communicate impressions of cases to appropriate staff
- Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers
- Wear multiple hats as operations develop
- Travel within the state of Indiana (reimbursement for mileage and expenses)
Ideal Candidate Will Possess:
- 2+ years of experience working with frail or elderly disabled adults and caregivers
- Previous field experience in a medical or case management environment, preferred
- Experience with data input, case review/case note entry; Salesforce experience is a plus
- Hands-on experience in home care, preferred
- Working knowledge of ADL/ IADL assessments
- Valid driver’s license with an insured vehicle for travel within the state
- Excellent verbal and written communication, organizational and time management skills
- Strong critical thinking and problem-solving skills
- Proficient in Microsoft (i.e., Word, Excel, Outlook)
- Excellent training skills
- Team-player mentality

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $22.00 and $26.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
About this role
FreedomCare's Care Manager role in South Bend is a field-based position centered on patient intake, caregiver training, and ongoing case oversight in home care settings. You'll conduct initial home assessments to evaluate client needs and caregiver fit, lead orientations with new patients, train caregivers and clients on care protocols, and make periodic supervisory visits to monitor active cases. The work involves collaboration across an interdisciplinary team, documentation in case management systems, and the flexibility to adapt as operations evolve—all while traveling throughout Indiana with mileage reimbursement.
The ideal candidate brings at least two years working directly with elderly or disabled adults in a medical or case management field setting.
