About this role
The Jane Pauley Community Health Center is looking for someone to serve as the first point of contact for patients navigating healthcare access and community resources. As a Community Resource Navigator, you'll work across multiple clinic locations in an ambulatory care setting, helping individuals and families understand their benefits options, complete applications for Medicaid and Medicare, and connect with assistance programs. The role requires you to be comfortable working outreach events at varying times and locations, managing a food pantry, and addressing barriers that prevent people from using JPCHC services—all while maintaining strict confidentiality and treating vulnerable populations with genuine respect.
Your day-to-day work will blend administrative tasks with direct patient support: processing sliding fee scale applications, researching insurance issues, educating patients about their coverage, and following up on government assistance requests. You'll also participate in community health events and help address social determinants of health. The position floats across clinic sites, so you'll need a valid Indiana driver's license and willingness to travel 50% of the time.
To succeed here, you need customer service experience, a high school diploma, and proficiency with Microsoft Office and Adobe Acrobat. You'll be required to obtain three certifications within your first six months: Indiana Navigator, Certified Application Counselor, and Community Health Worker. Bilingual or multilingual skills are valued. The center offers medical, dental, and vision coverage, generous PTO, a 401(k) with employer contribution, and life and disability insurance.
