About this role
The Jane Pauley Community Health Center is looking for a Behavioral Health Therapist to work with an established patient panel in Kokomo, serving a medically underserved community. You'll conduct psychosocial assessments, provide individual and family counseling, and respond to crises while embedded in an integrated care model alongside physicians, nurse practitioners, care managers, and outreach specialists. This is fundamentally collaborative work—you'll participate in team meetings, coordinate transitions across care levels, and build relationships with community resources and protective services to ensure continuity of care.
The role requires an active Indiana license (LCSW/LSW, LMHC/LMHCA, or LMFT/LMFTA) and a master's degree in social work, counseling, or marriage and family therapy. You'll need solid grounding in crisis intervention and de-escalation techniques, knowledge of mandatory reporting requirements, and the ability to document thoroughly in an EHR. CPI certification is preferred. This position suits someone with healthcare or community mental health experience who is energized by working in underserved settings and values mission-driven work over a traditional clinical practice.
The center offers medical, dental, and vision coverage, generous PTO, a 401(k) with employer contribution, and life and disability insurance. This is a full-time, in-office role in Kokomo, Indiana.
