Description

Health Link Home Health & Hospice is seeking an experienced Registered Nurse (RN) to join our growing Transitions Department.

The Transitions Nurse works closely with our Home Health clinical teams to identify patients whose healthcare needs are changing and help ensure they receive the appropriate level of care, services, and support. This may include hospice, palliative care, additional caregiving support, higher levels of care, placement, or other community resources based on the patient’s individual needs.

The Transitions Nurse serves as a consistent resource for patients and families throughout these changes, providing education, guidance, coordination, and follow-up to help ensure patients receive the right care at the right time.

A key component of the role includes identifying patients who may be appropriate for hospice, providing hospice education and informational visits, and facilitating timely transitions when hospice becomes the appropriate level of care.

JOB REQUIRES ON-SITE AND FIELD-BASED WORK

  • Educate Home Health clinical teams on hospice appropriateness, indicators of decline, changing patient needs, and when additional services or a different level of care should be considered.
  • Provide hospice education and in-services to Home Health teams and community healthcare partners.
  • Collaborate with physicians, nurses, therapists, social workers, Hospice staff, and other healthcare professionals to support appropriate and timely transitions.
  • Serve as a consistent point of contact for patients and families throughout changes in their healthcare needs.
  • Maintain accurate and timely documentation of patient and family communication, follow-up activities, resources provided, and transition status.

About this role

Health Link Home Health & Hospice is hiring a Transitions Nurse to help patients and families navigate the move from home health care into hospice. This role sits at a critical juncture in patient care—you'd assess patients' physical and emotional needs, educate families about what hospice entails, and coordinate with home health teams and physicians to ensure smooth handoffs. The work combines clinical assessment with substantial patient advocacy and education, including leading informational sessions for families and in-services for staff.

You'll need a California nursing license and at least one year of nursing experience, ideally in home health, palliative, or hospice settings. The role demands strong communication skills and comfort working both independently and within a multidisciplinary team. This is an on-site position in San Francisco, so you'll be meeting patients and families in person as a primary point of contact during a vulnerable time. The pay ranges from $55 to $70 per hour depending on experience, with standard benefits including health insurance, 401(k) matching, and paid time off.

This role suits nurses drawn to end-of-life care who want to focus on the human side of transitions—education, support, and coordination—rather than direct clinical care alone.
