About this role

Walker Meadow Retirement Community in Grand Rapids is seeking an LPN for per diem shifts at their long-term care facility. You'll provide direct nursing care to residents—administering medications, monitoring vital signs, managing wound care, and conducting health assessments. Beyond hands-on clinical work, you'll coordinate with interdisciplinary teams, mentor support staff, and maintain comprehensive documentation while ensuring infection control and safety protocols are followed.

This role suits an LPN with geriatric or skilled nursing experience who can work independently and lead a care team. You'll need current Michigan LPN licensure and BLS certification; a background in long-term care is preferred. The position requires the ability to lift up to 35 pounds and flexibility to pick up a minimum of three shifts monthly, with day shifts (6am–6:15pm) and night shifts (6pm–6:15am) available. Night and weekend differentials apply, and the base rate of $35.23–$41.60/hour includes a $3.40/hour pass-through supplement through September 2027.

The per diem structure offers flexibility alongside benefits including tuition assistance, a wellness reimbursement, employer retirement contributions, and daily pay transfers. A $500 referral bonus is available if you bring new team members on board.
