About this role

VitalCaring is seeking an RN to serve as Clinical Care Manager in their Ocean Springs home health operation—a role that positions you as the clinical leader coordinating care across patients, providers, and internal teams. You'll manage the full patient journey from intake through discharge, handling care coordination, EMR documentation, physician communication, and compliance while making independent clinical decisions that directly shape outcomes. The position requires managing multiple priorities in a fast-paced environment, conducting assessments and case conferences, and supporting care delivery through in-home visits as needed.

This role suits an experienced home health clinician with strong organizational skills and the confidence to work autonomously while collaborating across disciplines. A current RN license and BSN are baseline requirements; home health experience and background in care coordination or clinical leadership are strongly preferred. You'll need solid clinical judgment, attention to documentation detail, and the ability to identify and act on issues proactively—balancing accountability with compassion in a field-based setting.

VitalCaring offers a full-time salaried position with genuine flexibility, comprehensive medical and mental health benefits, 401(k) matching, and generous PTO. The company emphasizes professional development through tuition reimbursement and positions this as a growth-focused culture where your leadership directly influences patient care quality.
