About this role

Ascension's rural primary care practice in Chilton offers a full-time position for a Family Medicine physician to lead clinical care in a high-volume clinic serving approximately 4,000 patients. You would join an established team of one physician and five advanced practice providers, with a 1:7 call structure, working on campus at Calumet Hospital—a 2024 Top 100 Critical Access Hospital. The role centers on comprehensive patient evaluation, diagnosis, and treatment, with opportunities to participate in quality improvement initiatives and inpatient/outpatient consultations within Ascension's broader health system network.

The position suits physicians seeking practice stability and local autonomy within a larger nonprofit system. You'll work in a collaborative, team-oriented environment with built-in support staff and the resources of a national Catholic health system, while maintaining the independence that comes with a physician-led culture. Candidates must hold or be eligible for Wisconsin or Minnesota medical licensure and DEA registration, with an MD or DO degree required.

Chilton's location provides small-town living with access to outdoor recreation around Lake Winnebago and the Manitowoc River, while remaining within reasonable distance of Appleton and Green Bay. This is an employed position offering the stability and benefits of a established health system without the administrative burden of private practice ownership.
