About this role
Ascension's Family Medicine position in Milwaukee places you in an outpatient primary care practice embedded within an Internal Medicine residency program at Columbia St. Mary's Hospital. You would manage your own patient panel autonomously while working alongside an established team that includes an independent APP and RN triage support. The clinic serves a predominantly Medicaid and Medicare population with significant social determinants of health, offering the chance to deliver meaningful, high-value care to a community that has long relied on this practice as a healthcare anchor.
Beyond direct patient care, the role involves coordinating care transitions—managing specialist referrals, inpatient admissions, and discharge planning in collaboration with case management. You'll also participate in or lead quality improvement initiatives aimed at enhancing efficiency and cost-effectiveness across the clinic and wider system. Complete, timely documentation and EHR maintenance are essential to ensure compliance and practice financial integrity.
You'll need an MD or DO, Wisconsin medical licensure obtained before your start date, and Advanced Life Support certification (American Heart Association or American Red Cross). Ascension emphasizes physician-led collaboration, long-term practice stability, and mission-centered care within a national nonprofit Catholic health system that values local autonomy.
