About this role
Ascension's Family Medicine practice in Chilton offers a full-time outpatient role for an NP or PA in a rural primary care setting with meaningful clinical autonomy and institutional support. You'd join an established team of one physician and five advanced practitioners, seeing 12–15 patients daily in a high-volume clinic located on the Calumet Hospital campus—a 2024-recognized Top 100 Critical Access Hospital. The work centers on comprehensive patient care: conducting assessments, ordering and interpreting diagnostics, managing acute and chronic conditions, and delivering preventive services alongside patient education and care planning in collaboration with physicians.
This position suits clinicians drawn to rural practice who value both clinical stability and community connection. Ascension's national network provides resources and referral pathways while maintaining local autonomy, and the practice operates within a collaborative, team-oriented culture. The call structure is 1:7, and you'd be serving a population of approximately 4,000 in the scenic Chilton area, with proximity to Appleton and Green Bay plus access to outdoor recreation around Lake Winnebago and regional trails.
Candidates must hold Wisconsin NP licensure and RN credentialing prior to hire, a Master's degree in Nursing, and BLS certification (DEA credentialing preferred). This is a straightforward clinical role within a stable Catholic health system known for supporting its clinicians long-term.
