About this role

At Ascension's Mequon location, this role oversees a team of case managers and clerical staff responsible for managing patient transitions, disease management, and coordination between hospital administration and medical staff. You'll handle daily scheduling and workload distribution, serve as the go-to expert on utilization criteria and documentation standards, and step in directly on complex cases as needed. The position bridges clinical operations and strategic planning, requiring you to communicate discharge planning decisions and length-of-stay appropriateness across departments.

You'll need an active Wisconsin license as a registered nurse or social worker, or ACMA/CCMC case management certification. Beyond the required three years of experience, at least one year in a leadership or management role is essential. A master's degree is preferred but not required if you have the foundational education and experience. This is an in-office, full-time position that demands both administrative acumen and hands-on clinical judgment.

Ascension offers comprehensive benefits including medical, dental, and vision coverage; employer-matched retirement contributions; tuition assistance through its Vocare program; and professional development support. The organization emphasizes career growth and emotional well-being resources alongside standard paid time off and family support options.
