About this role
Ascension's Registered Nurse Case Management role in Indianapolis centers on orchestrating patient care from hospital admission through discharge or transition to the next care setting. You'll work across clinical teams, families, and insurance partners to design coordinated discharge plans, identify and remove barriers to recovery, and ensure patients have the resources they need when they leave the hospital. The position also requires continuous evaluation of medical necessity and care levels to balance quality outcomes with appropriate resource use.
This is a full-time, in-office position that demands at least two years of acute care inpatient nursing experience and a current RN license. Case Manager certification from CCMC or ACM is preferred but not required at hire. The role suits nurses with a background in acute care who are comfortable navigating complex logistics, advocating for patient needs, and collaborating across multiple stakeholders to prevent gaps in post-discharge care.
