About this role

Ascension's Social Worker Case Management position in Milwaukee centers on designing and coordinating individualized care pathways for patients navigating complex health and psychosocial challenges. You'll work across the full continuum of care, evaluating needs that span substance abuse, mental health crises, legal barriers, and adjustment difficulties, then advocating fiercely to connect patients with the resources and services they need. This role demands both clinical judgment and systems navigation—identifying what's blocking a patient's progress and securing the healthcare and community support to move them forward.

Success here requires Wisconsin licensure or certification as a Licensed Clinical Counselor, Licensed Social Worker, or Case Manager (CCMC preferred), along with relevant clinical or case management experience. You'll spend your time in dialogue with supervisors, care transition teams, and other clinicians, ensuring treatment plans are grounded in patient and family values and that documentation reflects accurate levels of care and diagnostic reasoning. The work is fundamentally about enabling smooth transitions and post-discharge stability through coordination, advocacy, and continuous collaboration within Ascension's health system.
