Liquid Personnel is recruiting Occupational Therapist for its client's Reablement Team Wiltshire.
Our client is looking for an experienced Occupational Therapist to join a supportive Reablement Team, helping adults with physical disabilities, long-term conditions, autism and sensory impairments to maximise their independence.
What will your responsibilities be?
- Complete OT assessments and reviews.
- Develop person-centred reablement plans.
- Assess functional needs, risks and home environments.
- Recommend equipment and adaptations.
- Promote independence and improve quality of life.
- Work closely with social care, health and multidisciplinary teams.
- Maintain accurate records and contribute to safeguarding and risk management.
Qualifications & Experience:
To be successful in this role you must have,
- HCPC registered Occupational Therapist.
- Experience within a community, social care or reablement setting.
- Strong assessment and clinical reasoning skills.
- Knowledge of equipment, adaptations and moving & handling.
- Excellent communication and partnership-working skills.
- A person-centred approach with a passion for supporting independence.
