About this role

Accompany Health is building a care model centered on patients with complex medical and social needs, and this role puts you directly in that work. As a Health Advocate, you'd be the primary point of contact during home visits—facilitating virtual appointments with clinicians, collecting vitals and administering behavioral health screenings, identifying social barriers like housing or food insecurity, and coaching patients through their care plans. You'd work across an interdisciplinary team and use technology to keep patients connected to care while helping them avoid unnecessary hospitalizations.

The role demands someone comfortable working independently in patients' homes, able to build trust quickly with people who may be hard to reach, and skilled at managing multiple priorities in a fast-moving environment. You'll need a patient-facing healthcare background—as a Medical Assistant, Community Health Worker, Health Coach, or similar—plus a valid driver's license and reliable vehicle. Basic clinical competency (vitals, screening tools) and comfort with mobile apps and video platforms are essential. Bilingual skills in Spanish or Arabic, prior experience in home-based or virtual care models, and formal certification (CMA or CHW) would strengthen your candidacy.

This is a full-time, in-office position in Detroit paying $22–$25 per hour. It's suited to someone who wants hands-on impact in primary care transformation and thrives working with vulnerable populations in their own environments.
