About this role
Mission Healthcare operates one of the largest home health networks in the western US, and this Phoenix-based role offers the chance to deliver direct patient care in a hybrid setting with a structured Monday-through-Friday schedule in the Scottsdale and central Phoenix area. You'd evaluate patients' physical conditions, design and implement individualized treatment plans, and educate both patients and caregivers on exercises and self-care strategies—all while collaborating with an interdisciplinary team to support patients across the healthcare continuum.
The position requires an active PT license, a master's or doctorate in physical therapy, and at least one year of experience, with home health background strongly preferred. You'll need current BLS certification, a valid driver's license, and a reliable vehicle for in-home visits. The salary ranges from $120–$130 depending on experience, with a $10,000 sign-on bonus, and the company offers comprehensive benefits including medical, dental, and vision coverage for full-time employees, plus tuition reimbursement and mileage reimbursement.
This role suits someone ready to transition into or deepen their home health practice—the hybrid arrangement and predictable schedule provide stability, while the interdisciplinary model and patient education focus appeal to clinicians who value holistic, relationship-based care. Mission Healthcare emphasizes its commitment to supporting staff well-being and building an inclusive workplace, which may resonate if you're looking for an employer that prioritizes both employee and patient experience.
