About this role

Freedom Care is seeking a part-time Registered Nurse to serve as a Care Manager for their home care operations in the New Haven area. This field-based role involves traveling to patients' homes throughout Connecticut to conduct initial clinical assessments, develop individualized care plans, and provide ongoing care coordination. You'll work directly with patients, caregivers, and interdisciplinary teams to monitor health outcomes, ensure regulatory compliance, and identify opportunities to improve safety and quality of care.

The position centers on clinical assessment and care management rather than direct patient care. Your responsibilities include reviewing caregiver documentation daily, coaching caregivers on best practices, coordinating with physicians and community resources, and maintaining detailed case summaries. You'll need a valid Connecticut RN license, a reliable vehicle for frequent home visits, and proficiency with case management software and standard office applications. New graduates are welcome to apply, though prior experience in home care or care coordination is valued.

This role suits an RN who thrives in independent fieldwork, enjoys patient education and advocacy, and is comfortable managing multiple cases with minimal on-site supervision. The part-time arrangement offers flexibility while the salary range is $45–$50 per hour.
