About this role
MedElite Group is hiring an LPN to manage behavioral health care for residents across skilled nursing and long-term care facilities as part of a Collaborative Care Model program. Working in-office in Hollywood, Florida, you would support patients remotely through medical record review, behavioral health screening, care planning, and coordination with an integrated team of providers, psychiatrists, and facility staff. The role centers on identifying clinical concerns, documenting standardized assessments like PHQ-9 and GAD-7, developing individualized care plans, and preparing organized case summaries for psychiatric consultation meetings.
This position requires strong clinical judgment, documentation skills, and the ability to manage a high-volume caseload while meeting strict deadlines and compliance standards. You would not be diagnosing or prescribing independently; instead, you would escalate findings to the treating provider and psychiatric consultant for clinical decision-making. The work demands proficiency with electronic medical records, excellent communication across multiple care teams, and the ability to recognize and appropriately flag behavioral health, medication, and safety concerns.
The role suits an LPN with solid experience in clinical documentation, medical-record review, and care coordination who is comfortable working in a structured, metrics-driven environment. You should be organized, detail-oriented, and able to balance competing priorities while maintaining accuracy and compliance. The salary range is $64,000 to $70,000 annually for full-time, in-office work.
