About this role

Otterbein SeniorLife is seeking an MDS Nurse Transition Leader to fill interim leadership gaps across its senior care communities in Ohio and Indiana. You'll carry full MDS nursing responsibilities at assigned locations while also leading transitions, managing care plans, facilitating utilization reviews, and serving as a therapy liaison. When not in an interim role, you'll take on special projects assigned by the VP Clinical Quality. This position requires significant travel—up to 100% depending on transition needs—with lodging provided for commutes over an hour.

The role demands at least five years of MDS experience in long-term care, a current Ohio RN license (Indiana license a plus), and proven expertise in casemix and PDPM management. You'll need strong documentation and communication skills, comfort with electronic medical records, and the flexibility to adapt quickly across different Otterbein settings. The ability to handle the emotional demands of working with elderly and ill residents is essential, as is attention to detail in compliance and quality measures.

Otterbein offers comprehensive benefits including medical, vision, and dental coverage; a 401(k) with company match; immediate PTO accrual; tuition reimbursement up to $5,250 annually for any field of study; and access to professional development resources. As a nonprofit organization, Otterbein emphasizes person-centered care and community, making this a fit for nurses seeking meaningful work in senior services.
