About this role
Alpine Care Home Health is seeking an Occupational Therapist to deliver skilled home-based care to patients in the Cleveland area. You'll work directly with patients in their homes, conducting functional assessments, developing and adjusting treatment plans under physician direction, and guiding patients and families through therapeutic activities designed to restore independence. The role involves comprehensive documentation, regular reassessment against established goals, and close coordination with the broader care team—physicians, nurses, and other specialists—to ensure holistic, evidence-based treatment aligned with each patient's priorities.
This position requires a state license with at least two years of OT experience, active CPR certification, and a reliable vehicle for travel between patient visits. You'll supervise occupational therapy assistants as needed, participate in staff development, and maintain a 90% productivity standard. The work demands strong clinical judgment, clear communication with families and team members, and the ability to adapt treatment approaches based on patient progress and changing conditions.
The role suits someone drawn to direct patient care in a less institutional setting, who values autonomy in clinical decision-making and finds purpose in helping people maintain safety and independence at home. Alpine offers competitive salary, health benefits, 401(k) matching, mileage reimbursement, and a collaborative, mission-driven environment.
