What You'll Do
In this role, you will design, develop, and deploy robust software solutions to securely access, transfer, and transform raw patient data into high-quality, ready-to-use real-world evidence and data products. You will collaborate with Flatiron colleagues globally and work day-to-day with oncologists, clinicians, product leaders, quantitative scientists, and customers to understand their challenges and build technological solutions. In addition, you will:
- Drive key technical objectives within the team's roadmap and lead architectural decisions for your areas of scope, ensuring alignment with global Flatiron engineering standards.
- Lead the architecture, design, and development of data extraction and web applications that meet the needs of a rapidly changing business
- Devise creative solutions to solve tough business, security, and data privacy challenges
- Lead the hands-on deployment of solutions at hospital partner premises, directly navigating and troubleshooting complex local infrastructure, network configurations, and technical constraints.
- Mentor and guide other engineers, fostering a culture of technical excellence and continuous learning.
Who You Are
You're a kind, passionate and collaborative problem-solver who values the opportunity to think beyond the way things are. In addition:
- You have 5+ years of experience with software development using programming languages, such as C#, Java or Python
- You have 5+ years of experience with web technologies (such as HTML, CSS and JavaScript) and backend frameworks (such as ASP.NET Core and Spring MVC)
- You have 3+ years of experience with relational database systems, such as PostgreSQL and MySQL
- You have 3+ years of experience with Linux system administration
- You have experience working with infrastructure as code (IaC)
- You have deep expertise in system architecture, data modeling, and designing secure APIs.
- You have business-level proficiency in English
- You have native or advanced business-level Japanese proficiency, with the polished communication skills and professional etiquette required to lead technical discussions, align on requirements, and build trust directly with physicians, hospital executives, and vendor partners
- You have a strong understanding of networking fundamentals (TCP/IP, DNS, routing, firewalls, and proxy configurations) and experience troubleshooting connectivity issues in highly secure or restricted enterprise environments
- You are willing to travel to hospital partner premises to perform server operations when needed.

About this role
Flatiron Health is seeking a Senior Software Engineer to join its Tokyo office and contribute to transforming cancer care through real-world evidence. In this role, you'll design and deploy software systems that securely extract, transfer, and transform patient data into actionable insights, working across the full stack from data pipelines to web applications. You'll partner with oncologists, clinicians, data scientists, and hospital partners globally to understand their needs and build solutions that meet rigorous security and privacy standards.
The position demands deep technical expertise: 5+ years developing with C#, Java, or Python; 5+ years with web technologies and backend frameworks like ASP.NET Core or Spring MVC; 3+ years with relational databases and Linux administration; and proven experience with infrastructure as code and secure API design. Beyond coding, you'll lead architectural decisions, mentor engineers, and take ownership of deployments at hospital sites, troubleshooting complex local infrastructure and network constraints. A critical requirement is advanced business-level Japanese proficiency—you'll lead technical discussions directly with physicians and hospital executives, requiring polished communication and professional judgment.
This is a full-time, in-office role in Shinjuku with standard Tokyo hours (9:00–18:00, flex-time eligible) and statutory benefits. Travel to hospital partner premises for server operations is expected. Experience with EHR systems, server hardware management, or AWS development is valued but not required.
