Position: Licensed Vocational Nurse (LVN)
Reports To: Team Leader
Hours: Monday to Sunday 5 pm to 8 am & Monday to Thursday 5 pm to 8 am
Compensation: $10 on call rate follow per diem visit rate
About the Role
We’re looking for an per diem LVN who is compassionate, reliable, and passionate about patient care. In this role, you’ll provide direct care under the guidance of a Registered Nurse, follow the plan of care, and work closely with our interdisciplinary team to ensure each patient receives the support they need.
What You’ll Do
Provide hands-on care to patients as allowed by the State Nurse Practice Act
Carry out care plans based on assessments and patient needs
Document visits clearly and on time
Help educate patients and families so they feel informed and supported
Communicate patient needs to the wider hospice team (aides, therapists, social workers, dietitians, supervisors, etc.)
Use equipment and resources safely and responsibly
Support pain and symptom management, and monitor how patients respond
Participate in training, development, and quality initiatives
Assist with other tasks as requested by the RN
What We’re Looking For
Graduate of an accredited LVN/LPN program
At least three years of nursing experience (hospice, community health, or med/surg is a plus)
Active LVN/LPN license in the state
Strong communication and organizational skills
Valid driver’s license and a reliable, insured vehicle
- Must be willing to travel far distances
Current CPR certification
A genuine understanding of the hospice philosophy and the needs of the terminally ill
About this role
Copper Sky Hospice is seeking a per diem LVN for overnight and evening shifts to deliver direct patient care in a hospice setting across the Phoenix area. You'll work under RN supervision, executing individualized care plans, managing symptoms and pain, documenting visits, and coordinating with an interdisciplinary team including aides, therapists, social workers, and supervisors. The role emphasizes patient and family education, clear communication about patient needs, and safe use of equipment and resources.
This position suits an experienced nurse with at least three years in nursing—ideally in hospice, community health, or medical-surgical settings—who holds an active state LVN/LPN license and current CPR certification. You'll need a valid driver's license, reliable insured vehicle, and willingness to travel significant distances throughout the Phoenix area. The work is per diem with a $10 on-call rate and per-visit compensation; shifts run 5 pm to 8 am, varying between Monday–Sunday and Monday–Thursday depending on the week.
Success in this role requires genuine commitment to hospice philosophy and the dignity of end-of-life care. You should be compassionate, reliable, and organized, with strong communication skills and the ability to work collaboratively across multiple disciplines to support patients and families through a sensitive time.
