About this role

Phoenix Med Health is seeking someone to build and manage the referral network for Active Vital Care, their in-home medical provider serving assisted living facilities, group homes, and private residences. You'd own relationships across multiple referral channels—facilities, home health agencies, hospice providers, and similar partners—with the core mission of driving consistent patient volume and new registrations. This is fundamentally a relationship and pipeline management role: regular in-person visits, consistent follow-up, quick issue resolution, and internal coordination with scheduling and clinical teams to ensure partners stay engaged and satisfied.

The role suits someone with at least two years in healthcare account management, business development, or referral management who's comfortable working in the field and managing a high-volume partner pipeline. You'll need strong communication skills, the ability to build trust quickly with both clinical and administrative stakeholders, and a track record of turning relationships into measurable outcomes. Experience with facilities, home health, hospice, or similar referral environments is expected; familiarity with CRM tools, EMRs, or the Medicare and senior care landscape is a bonus.

Success is tracked clearly: growth in active referral partners, monthly new patient volume, referral activity per partner, and retention of key accounts. You'll be compensated on an hourly rate plus performance incentives tied directly to referral growth and account performance. This is an in-office role based in Phoenix, Arizona, designed for someone who takes ownership, thinks in terms of growth and accountability, and thrives on execution.
