You will promote person-centered care, ensure regulatory compliance, supervise staff, and partner with families and interdisciplinary teams to support residents’ physical, emotional, and psychosocial well-being. In the absence of the Director of Health and Wellness, you will assume full operational oversight of community health services and wellness programming to ensure continuity of care and compliance.

KEY RESPONSIBILITIES

• Serve as the clinical leader, supporting the care team and overall healthcare operations of the community. • Collaborate with clinical and non-clinical staff to meet the healthcare needs of residents. • Hire, train, supervise, and manage caregiving staff, including caregivers and medication technicians.

• Ensure all healthcare operations comply with DSS regulations, including incident reporting and required in-service training programs.

Requirements

• Develop, implement, and maintain individualized care and service plans, including updates based on changes in resident condition. • Monitor and anticipate changes in resident status and ensure appropriate interventions. • Lead clinical quality and process improvement initiatives through tracking, trending, and analysis of internal data.

• Oversee contracted pharmacy operations, ensuring compliance with medication management regulations, including storage, administration, and documentation. • Coordinate with external healthcare providers, including hospitals, physicians, home health agencies, hospice providers, and specialty clinics. • Represent the community as a healthcare professional in external partnerships and professional interactions.

• Coordinate with internal transportation services to support resident medical appointments. • Respond to emergencies in accordance with established emergency management procedures. • Participate in overall community operations and events as needed.

• Serve as a liaison to residents’ families, addressing concerns, requests, and care needs. • Collaborate with caregiving staff to develop and manage staffing schedules. • Provide direct clinical and caregiving support to residents when needed.
