Key Responsibilities
The Hospital In-Patient Specialist is responsible for obtaining and communicating a daily disposition for all patients in an acute or post-acute rehabilitation setting.

are completed with limited supervision and may involve medical and social service activities and correspondence. This position requires initiative, creative problem resolution, time management, excellent phone and electronic communication skills, service excellence, and a desire to work within a practice that centers around and with a geriatric population. In this role you will be responsible for: Serves as a resource person regarding available health and social services and resources to the patients, caregivers, office staff, and providers Follows all hospital in-patient patients through admission to discharge Alerts schedulers to Hospital Discharge for follow-up with necessary TCM visits Maintains patient medical records.
Required Qualifications
High School Diploma or GED CNA or MA certified or equivalent work experience Minimum 1 year of experience in the medical field, medical office setting, or related position Desired
