Requirements

About The Role

: Phamily is assisting The NeuroMedical Center in hiring a Chronic Care Coordinator to work on-site using the Phamily Chronic Care Management platform. This role focuses on gathering and organizing patient data to identify unmet needs, engage patients in their care, and improve communication between patients and their care teams. Each Care Manager is expected to handle a 500-patient caseload with 300 billable patients by the end of the month.

Key Responsibilities
