BAYADA Home Health Care is seeking a Habilitation Manager to join our Erie, PA Habilitation Home Care office. If you are looking for an exciting career opportunity in a growing industry, a Habilitation Manager could be the position for you!

Responsibilities

for the Habilitation Manager: Act as internal case manager and advocate for clients in the Habilitation program within Erie, PA and surrounding areas. Participate in Person-Centered Planning processes Participate in an on-call rotation every 6 weeks, Monday-Monday.

Qualifications
